Dear Charlestown Resident,

Your safety and security is the primary responsibility for the Charlestown Security and Emergency Services Team. An important goal of ours is to ensure we can respond to your needs timely and responsibly in the event of an emergency. In that regard, we are pleased to introduce you to our new “File of Life” program. This will replace our existing Vial of Life program and we will be more user friendly and easier to maintain. The information you will provide is vital to you and to any first responder assisting you.
Enclosed is a new form for you to complete and a pouch that the form goes in.

  • Please complete all of the blocks and provide the most up-to-date information.
     
  • Once you complete the form, place it; and any M.O.L.S.T./DNR Directive, in the pouch and place it on the outside of your refrigerator.

By placing the pouch with the information in it, on the outside of your refrigerator, our Security & Emergency Services (SES) Officers can easily find it should the need arise. By being displayed on your refrigerator, it should be a reminder to you to update your information as the need arises.
Once you have placed the completed the File of Life form in the pouch on your refrigerator please dispose of the old Vial of Life and its contents. Please ensure you tear up your old forms.
