Have you and your family “had the talk”… about handling your Charlestown affairs at the end of life?

Important Information:

  • Ensure your family has a copy of your Will or Trust.
  • Veterans, make sure a copy of your DD-214 is available.
  • Review the Residence and Care Agreement and Beneficiaries on the Refund Form- with your family, which details your Entrance Deposit refund.
  • Ensure that the Authorization for Entry to Living Unit form is completed

Getting Your Affairs in order:

When the final surviving resident passes in the Independent Living apartment, Security will place security lock on the door of the Unit to prevent unauthorized access.

An “Authorization for Entry to Living Unit” form must be completed which allows:

  • Up to two authorized persons to obtain keys to the unit
  • The form can be obtained from the Administration Office or the Resident Life Office
  • The form needs to be filled out and given to our Resident Life office at- Cross Creek, 719 Maiden Choice Lane, Catonsville, MD 21228, 410-737-8838 Ext.8230

If there is no Authorization for Entry form completed, the Executor or Personal Representative of the estate must obtain a “Letter of Administration”, from Baltimore County Courthouse-Register of Wills in Towson, MD, for relatives to gain access to the residence. If a Trust is involved, the family will need to provide the Trust forms to Security for approval.

We suggest that you call ahead to the Register of Wills office at (410-887-6680) to find out exactly what documents are required by the county to receive a Letter of Authorization. Examples:

  • Copy of the original Will
  • Death Certificate
  • Amount of money going into the Estate, which is listed in your Resident’s Care Agreement
  • Additional documents may be required

If and when necessary, Security will escort your representative into your apartment to access any needed documents. The Executor (or Trustee) of the Estate should have a copy of the Trust or the Letter of Administration for Security, in order to receive the key to the Security lock. We suggest making 12 copies of the Death Certificate to notify any related organizations, etc.

Treasure Sale is asking us to remind families that we offer our volunteers to pack up any donations left behind in loved ones’ apartments and that there is no charge for the approved removal of larger items left for donation to the Treasure Sale. Emily Fowler is their contact at 410-737-8892.

Follow Up

Your family should contact the Move-Out Coordinator, Bobbi Reynolds, to obtain a copy of the Move-Out Handbook, which will guide them through the Move Out process. Our goal is to assist your family through this difficult time, to expedite the unit turn-over quickly.
